If you have spent your own money (within your budget) for an HSA program and are owed reimbursement, please fill out this HSA Check Request and Reimbursement Form. 

Please follow all instructions, including sending a copy of your receipt to the HSA Treasurer's email:

Please allow 2 weeks for all requests.
